Abstract

This project is aimed how to apply Process FMEA (PFMEA) concept that use to control the reject number of product. Process FMEA (PFMEA) concept can be quality management tool that use to identify, analyzing and prioritizing failure modes of a product and process by improve the quality and productivity of the company. It also can help to reduce the risk of producing defective part in production line. The major problem could be happen in the process are human mistake and higher reject rate. Regarding to this factors, there are several problem to face everyday such as producing some defective product, waste material, time and money, also effect to the product quality. The best solution to improve the risk priority number (RPN) in Process FMEA is using the Standard Operation Procedure (SOP). The DMAIC method is used to implement the correlation between the SOP through PFMEA level. The production process becomes more reliable, tolerant and safe when FMEA is linked together with SOP. SOP is a very concept to be implementing by human because it can prevent the problem and reduce the defect of the product. If the SOP used properly and correctly, the product does not have any defect and no rejects products on the production. As a result of the application of SOP in the company, the RPN has been reduced by 82 percent which is 70 to 392 and 288 to 60. Defects also are reduced by making zero defects to the next month.
